Our duty is to help you control your health conditiones, so that you can stay happy and healthy, in your own home.
Chronic care management is a Medicare covered service where beneficiaries with 2 or more chronic health conditions are paired with a health coach to help improve the lives of the patient and improve health outcomes.
We work with your primary care provider and specialist to make sure that you stay healthy and stay home.
The health condition must be expected to last at least 12 months.
Some examples are:
Alzheimer's disease, Arthritis, Asthma, Autism, Blindness, Cancer, Cardiovascular disease, Deafness or hearing impairment, Diabetes, Endometriosis, Epilepsy, Fibromyalgia, Heart disease, High blood pressure, HIV/ AIDs, and many others.

Because this is a Medicare covered benefit, most patients have a $0 cost and some patients may have some sort of cost sharing but, be rest assured, you will not be required to pay for any services before eligibility is established!
