Remote Patient monitoring

Each device is free of charge as long as you are enrolled in the program. A Registered Nurse will come to your home, set your device up, and show you how to use it. After that, you are able to check your values and they will go directly to your health coach and provider. 

Remote Patient Monitoring

Many members enjoy the convience of not having to remember what their numbers are or having to bring the notebook, blood pressure cuff, or glucometer at every primary care appointment. It frees up time to speak with your provider about other pressing matters.
